Hoag’s Acute Rehabilitation Program has served 530+ patients with top-rated patient experience and discharge rates.

Rehabilitation after amputation helps patients regain independence, learn how to use prosthetics, and adapt daily activities to restore mobility, strength, and confidence.
Whether caused by trauma, accident, or illness, brain injuries can affect movement, memory, or speech. Acute rehabilitation focuses on restoring lost skills and helping patients relearn how to safely navigate daily life.
After brain tumor surgery, patients may need specialized therapy to address changes in balance, coordination, speech, or cognition, ensuring a smoother transition back to normal activities.
Cardiac rehabilitation helps patients recovering from heart conditions or surgery rebuild endurance, strengthen their cardiovascular health, and reduce the risk of future complications.
Conditions such as Parkinson’s disease, multiple sclerosis, or other movement and balance disorders benefit from targeted rehabilitation to improve mobility, reduce complications, and enhance quality of life.
Cancer and its treatments can lead to fatigue, weakness, or mobility challenges. Rehabilitation supports patients in regaining strength, managing side effects, and improving overall function.
From joint replacements to fractures, orthopedic rehab helps restore strength, flexibility, and movement while reducing pain and improving long-term outcomes after surgery or injury.
Rehabilitation for lung conditions such as COPD or pulmonary fibrosis focuses on improving breathing techniques, building endurance, and supporting safe activity levels.
Rehabilitation is essential after spinal cord injury or surgery to maximize recovery, restore function, and teach patients adaptive strategies for independence.
Stroke rehabilitation helps patients relearn essential skills such as speaking, walking, and using their hands, with therapies designed to restore independence and quality of life.
At Hoag, we understand that starting rehabilitation can feel overwhelming. That’s why we make sure every patient knows what to expect and feels supported every step of the way. While your treatment plan will be customized to your unique condition, recovery goals, and pace of progress, there are some common experiences all patients can anticipate during their stay.
Your journey begins with a thorough evaluation by our licensed admissions liaison and medical director. Together, they take the time to learn about your health history, current needs, and personal recovery goals. This assessment helps us design a customized care plan that balances short-term improvements with long-term independence. You and your family are active participants in this process, ensuring your treatment plan reflects what matters most to you.
Most patients participate in a combination of physical, occupational, and speech therapy for at least three hours per day, split between morning and afternoon sessions. Therapy is personalized to your needs—for example, building strength and balance after a stroke, relearning daily activities after orthopedic surgery, or strengthening communication skills following a neurological condition.
You will have a midday break for lunch and rest, and when appropriate, therapy may also be provided on weekends to help you continue making progress.
Your length of stay depends on your diagnosis and goals. Some patients may require a shorter stay, while others benefit from a longer course of therapy. On average, patients spend 12 to 14 days in our program. Throughout your stay, your care team will regularly review your progress with you and your family to update your plan and set realistic expectations for discharge and next steps.
During your stay, you’ll be supported by a compassionate, highly skilled team dedicated to your recovery. Every member of your care team works together to ensure your treatment is safe, effective, and personalized to your needs.
Physicians & Medical Director: Oversee your medical care, monitor your progress, and adjust your treatment plan as needed.
Registered Nurses: Provide around-the-clock medical support, administer medications, and help you manage daily health needs.
Physical Therapists: Focus on improving your strength, mobility, balance, and endurance so you can move with greater independence.
Occupational Therapists: Help you regain the skills needed for everyday activities such as dressing, cooking, writing, and managing your home.
Speech-Language Pathologists: Assist with communication, memory, and problem-solving, as well as swallowing challenges that may affect nutrition and safety.
Case Managers & Social Workers: Guide you and your family through the rehabilitation process, assist with discharge planning, and connect you to community resources for continued care at home.
Rehabilitation Nurses & Therapy Assistants: Support your day-to-day therapy activities and provide encouragement as you work toward your goals.
Together, this team creates a supportive environment where patients and families feel empowered, informed, and encouraged throughout the recovery journey.
We encourage you to bring a few items from home to make your stay more comfortable and help you participate fully in therapy:
Tennis shoes or workout-appropriate shoes (with interchangeable laces)
Comfortable, loose-fitting clothing that allows you to move freely
Undergarments and socks

Following a thorough assessment, our team discusses the patient's medical status and works with them to create a customized treatment plan based on their short-term and long-term goals. A detailed and comprehensive review of each patient’s condition and medical history is performed by our licensed Admissions Liaison and Medical Director to determine if the patient is an appropriate candidate for acute inpatient rehabilitation. The clinical team will evaluate whether or not the patient can tolerate and benefit from an intensive rehabilitation program.
Insurance verification and authorization are required before admission.

Recovering from a significant medical issue requires both supportive family and personal independence. Through participation in family conferences, education opportunities and rehabilitation sessions, families take a hands-on role in learning how to help set you up for success.
Our patient-centered program provides you with the tools to reach your goals and prepare your return to community living. You will wear your everyday clothes, eat in a large open dining room with other patients and work to develop functional independence in a compassionate environment. This is not acute hospitalization, it is rehabilitation. And while the road ahead takes work, our team strives to make that work rewarding.
Length of stay varies depending on your diagnosis. Our clinical team works with you to determine an estimate based on your plan of care. Average length of stay will range from 12 – 14 days.
