The Annual Health Assessment is a comprehensive visit that is covered under your insurance plan. During this visit, you will meet a Nurse Practitioner who works closely with your primary care provider as part of your care team at Hoag.
You are expected to begin Medicare on the first day of the month that you turn age 65, which may fall outside of the typical Open Enrollment period. You should receive your Medicare card in the mail 3 months before you turn age 65.
