Located within Hoag Hospital Newport Beach and Hoag Hospital Irvine, Hoag laboratories provide testing services for Hoag’s inpatient and outpatient population. Our dedicated team of over 230 licensed pathologists, clinical laboratory scientists, phlebotomists, and other laboratory professionals utilize state-of-the-art analytical instruments and technology to provide accurate and timely test results.
Hoag laboratories are accredited by the College of American Pathologists (CAP).

Without an Appointment: If you plan to walk in, you may check in at the lab front desk or the main registration desk.

Without an Appointment: If you plan to walk in, check in at the lab front desk on the 2nd floor. If you require other services during your visit, please check in at the main registration desk on the 1st floor.
The following documents are required to complete your tests:
Lab order form
Photo ID (driver license, passport, school ID, or other government issued ID accepted)
Health insurance card
Certain tests require fasting. The most common ones are the lipid panel, glucose, cholesterol, and triglycerides. Contact your ordering physician for specific instructions.
Two types of lab orders are accepted:
Always bring your order form with you, even if it has been sent electronically. If you have an appointment, you can ask your health care provider to fax your order form to our lab. This may allow us to prepare for your tests before your arrival, helping you save time. The following information is required:
Patient’s first and last name
Patient’s date of birth
Test name(s)
Diagnosis
Ordering physician’s name and signature
Your health care provider may be able to send us your order request electronically, helping you save time. Please request a printed copy of your order and bring it with you.
Making an appointment will help reduce your wait time. To make an appointment, call 949-764-5600 between 8 a.m. – 6 p.m. Monday – Friday.
Please have your lab order form and insurance card ready when you call.
PPO, POS, Medicare Parts A & B, and CalOptima Direct plans are accepted.

HMO plans are only accepted in certain cases:
1. Your HMO insurance provider has provided prior authorization.
Note: Hoag Outpatient Lab does not accept all HMO plans. Please contact your insurance company to check if prior authorization is required for testing at our lab.
2. You need lab tests performed prior to your surgery at Hoag and your lab visit is within three days of your surgery.
Note: Pre-surgical blood typing and screening (ABO & Rh Type, Type & Screen) and MRSA tests must be done at Hoag. You do not need to come within three days of your surgery for blood typing and screening or MRSA tests.

Your patient health records may be managed and accessed through MyChart. Only you, your provider and your health care team are permitted to view the information sent through MyChart. We use the latest security practices and safeguards to protect your health care information. MyChart is password-protected and delivered via an encrypted connection.

If you have an appointment, you must complete your lab tests at the location for which you scheduled your appointment. If you plan to walk in, you may complete your lab tests at either of our locations.

With an Appointment: Making an appointment will simplify the check-in process. A lab representative will review your order, verify your records, and order your lab tests.
Without an Appointment: A Patient Access representative will review your information to register you for your visit. Our lab representatives will then order your lab tests.
